Name and Contact Details

As a crucial part of the justice system, the role of Corrections Officer requires a high level of trust, responsibility and integrity.  The nature of the work could place staff in situations where their personal integrity may be challenged.  In order to ensure your safety and the safety of your colleagues, this form asks for detailed personal information.  Please fill in this form truthfully and accurately.  If you need help or further explanation with any questions in this form, please contact your local recruitment coordinator.

The information you provide will be used only for assessing your suitability as a Corrections Officer and for no other reason (Privacy Act 1993).  Please note that false information will disqualify your application from the selection process.


In this section you will be asked to complete your personal details such as name, address and contact details.  You can maintain these details yourself should they change in the future by logging back into the system (using your email address and password) and click on My History and then Edit Application.

Vacancy Statistics

In this section you will be asked to complete questions relating to your gender, ethnicity and how you found out about this vacancy.  This section is specifically for the use of gathering statistics.  The information is summarised and used to help us review implementation of our Equal Employment Opportunities and Recruitment Policies.  The information will not be used as part of the selection process.

In this section you will be asked questions relating to your legal right to work and drive in New Zealand as well as your first aid qualification.

As part of verifying these details, we require copies of the documentation that support your answers to these questions.  You can either upload scanned copies as part of your application or post them to us at the address in the job advertisement.  If you are uploading your documents, please upload your file in either .doc, .rft, .txt or .pdf formats.  Other formats may not be accessible by our technology which could delay the initial assessment of your application.

Physical Fitness

* Denotes a mandatory question
In this section you will be asked questions relating to any current or past medical conditions or injuries and if you are taking any medication.  The role of a Corrections Officer requires a standard level of physical fitness and as part of the selection process candidates are required to under go a physical fitness test.
Have you ever, or do you currently, suffer from any injury or illness, or medical or any other condition that may affect your performance in the job of a Corrections Officer?
Are you taking any drugs or medication that may affect your ability to perform the tasks of the position applied for?
Please provide the contact details of your regular doctor who can complete a medical examination to test your overall health, strength and stamina. Your doctor will need to provide clearance for you to participate in the separate physical test.  You will need to cover the cost of the medical, which may be funded by Work and Income subsidy or reimbursed if successfully appointed.  Please note that your ACC history may be checked with regard to your physical fitness. Referees

In this section we request the full names and contact details of two work-related and one character-related referee in support of your application.

Both work-related referees should be from your current or previous employment (i.e. your manager or supervisor) and should not include people with whom you have a close relationship. If you are self employed you may add the name and contact details for someone with whom you have a professional relationship, e.g. principle trade supplier, long term customer, accountant, lawyer etc. 

Your character-related referee should include someone outside of work who has known you for a minimum of two years and is not an immediate family member.

Criminal Convictions

Note: The positions of Corrections Officer or Probation Officer (and some related positions) are exempt from the provisions in the Criminal Records (Clean Slate) Act 2004. Therefore, the Department of Corrections is entitled to ask you to disclose all convictions when applying for these roles. A full list of the exempt positions is available in the Criminal Conviction Information Sheet, which will help you determine how to answer this question. If you are unsure please seek advice.

Please note that you must also declare convictions for which you received name suppression. This applies to all applicants, including overseas applicants.

All applicants will also be asked to complete a criminal conviction check that will be processed by the Ministry of Justice as part of the recruitment process.
